Ok, so that didn't work out as well as I thought it would, but you can still get the general idea. It's a flag.
Trinitas Insula means Triad/Trinity Island or Island Triad/Trinity. It is a small island nation off the coast of the Commonwealth that is dedicated to peace between human and nonhuman races.
H stands for humans
The S on the left stands for Synthetics (androids, robots, AI, etc.)
And the S on the right stands for Sc'alen (the first race on Sc'al to welcome the humans to the planet.)
As for the colors and shapes... I'm not quite sure why I added them. Feel free to let me know if they have any meaning to you.
